After you have read chapter 14 of your textbook, and reviewed the following report on Medical Errors: http://www.ahrq.gov/research/findings/factsheets/errors-safety/ (pay close attention to Chapter 2 – Efforts to Reduce Medical Errors: AHRQ’s Response to Senate Committee on Appropriations Questions) consider if you agree or disagree with the position that most of the medical errors are “systems” related and not attributable to individual negligence or misconduct? Why do you feel this way?.

According to the World Health Organization (WHO), the U.S. spends more on health services than most of the 191 countries evaluated in their annual report. The same report illustrates a ranking of 37 worldwide. If expenditures are high and patient outcomes are low, does this show inefficiency? Why or why not?
